APPLICATION DEADLINE IS MARCH 17, 2008.
Most common omissions that cause agencies to be denied:
- Missing signatures on the first and last pages of the application. Remember you must have the Authorization for Representation page notarized.
- IRS Form 990 not signed by Agency Executive on page 9
- No acceptable written plan to reduce administration and fundraising expenses to 25% or less
- IRS Form 990 and the Audit not for the same time period
- IRS Form 990 and the Audit not reconciled. Remember lines 12 and 17 from the 990 should match total revenues and expenses in audit or should be reconciled on page 4 of the 990. If not reconciled, it is necessary to obtain a signed reconciliation statement from the auditor who completed your audit
- Annual Report missing one or more of the following: full description of the agency's activities and supporting services; chief administrative personnel; board officers and directors identified
- Please note if terms of board, dates, times, and locations of meetings are not found in the Annual Report these items must be included as a separate attachment.
- Missing attachments. The Annual Report cannot be used in place of any other attachment. (For example, your board may be listed in the Annual Report, but you must also submit a separate list of board members.)
- United Way of King County MUST receive the most current Charitable Solicitation Letter from the Washington State Attorney General's office. If you do not have a charitable solicitation letter with a current date, submit the most recent one you do have AND a copy of your renewal application.
