APPLICATION DEADLINE IS MARCH 17, 2008.

• Application Form
• Authorization for Representation by United Way of King County (MUST be notarized and included with your application form).
• Acknowledgment of Non-Participation in UWKC Federation (MUST be returned if your agency chooses NOT to participate)
• Attachment C: A series of questions about the services or benefits your agency provided in 2007.
• Attachment H: Information for agency listings in campaign catalogs. This form MUST be included with the application. If no edits from the previous year are required, please check the box at the top of the page; otherwise, please edit. Note: you may check the previous year's catalogs listings at (State) or (Federal)
• Attachment K: City of Seattle non-discrimination policy (REQUIRED for City of Seattle campaign and must be printed on Agency's letterhead).
• Attachment L: Patriot Act which must be signed by agency Executive Director.
• Attachment M : IRS W-9 request for taxpayer identification number and certification.
